Depending on your Troop's activities, your daughter may be required to submit a variety of forms to her Troop Leaders. Below you will find some commonly used forms. Additional forms may be obtained from your Troop Leader.
Girl Health & Medical History Form - All Girl Members of AHG must complete this form prior to attending any Troop meetings or activities.
Adult Health & Medical History Form - All Adult Members and adults who volunteer and/or participate in Troop activities must complete this form and return to Troop Leadership.
High Adventure Activity Medical Form - All participants taking part in a "Restricted High Adventure Activity" must complete this form and return to Troop Leadership prior to activity. Restricted High Adventure Activities include, but are not limited to: boating, canoeing, sailing, cave exploring, downhill skiing, flying, horseback riding, whitewater kayaking, whitewater rafting, rock climbing, rappelling, and challenge courses.
Parent/Guardian Permission Slip - Must be completed and returned to Troop Leader prior to girl participation in any AHG Troop activity.
AHG Swim Test Record - This form can be used to verify a girl's swimming ability. It may be requested to participate in any AHG Troop swimming activity or High Adventure Water Activity such as canoeing, boating, or kayaking.
